Welcome to our online office forms. Here you can electronically submit forms needed to participate in some of our events. We also have physical copies of these forms for you to fill out at our location. The two immediate forms below is for non-members who will be attending a special event at Excite! such as Day Plays, Open Gyms, Girl Scouts, Recreational City League Teams, Recreational Cheer Teams, Birthday Party attendees and more!
A 30 day notice is required.
For short term breaks due to
vacations, holidays, etc.

All forms are submitted to our front office. Completing a form does not indicate final acceptance or confirmation.
You will be contacted via email and/or phone upon approval/confirmation for your request(s).
